A civilian in Middletown, New York, reported in April 1966 having seen a bright object the size of a golf ball. The object was flashing while rotating and remained visible for 3 to 5 minutes before disappearing to the west. Although the report included physical testimony, no photos were taken and no physical evidence was found. The Air Force requested more information to evaluate the observation, but the form was not returned.
The case was classified as "insufficient for evaluation" due to the lack of concrete data. The responsible officer, Major Levon Quintanilla, asked the witness to complete an additional form. Despite the report not providing definitive conclusions, the testimony was appreciated by military authorities.
