In July 1968, a witness in Sierra Vista, Arizona reported seeing a bright white object described as a "flying saucer." Although the observer was contacted to complete an official form, they did not do so before September 24 of that year. The lack of sufficient data prevented a conclusive evaluation by civilian authorities. The case was handled by Project 10073, an Air Force initiative aimed at collecting information on unidentified aerial phenomena.
The document includes a letter addressed to the witness, requesting them to complete an AF Form 117 to provide more details. The officer in charge, Lieutenant Colonel Rector Quintanilla, thanked the citizen for reporting their sighting. Despite the formal request, no further information was obtained from the observer, leaving the case unresolved. Cases like this, where information is incomplete or no response is received from the witness, were common during the investigations of that era, reflecting the difficulty of obtaining conclusive evidence about UFO sightings.
